2024 Living My Best Healthy Life Health Fair

Thank you for your interest in becoming a vendor at the 2024 Living My Best Healthy Life health fair. We are excited to have you!

The event will be held outdoors and is RAIN OR SHINE on Saturday, April 13, 2024 from 10 am- 1 pm. It is tentatively located at 3822 Robert L Parish, Dallas, TX.

Instructions and conditions for vendor setup as follows:

  • Setup for each vendor or individual service is booth space only that will allow the setup of a 6-foot table. Vendors are asked to provide their own table, tablecloths and chairs.
    Pop-up tents are allowed. If you plan to use a tent, let us know in advance.
  • All vendors should be event ready by 9:30 a.m. Plan to arrive between 8-9 a.m. to ensure a quality setup.
  • There is a $50 cost for the event. (note: registration payment option at the end of the registration form or you can contact our office for other options.)
  • Vendors are asked to provide one door prize item.
  • Frazier Revitalization prides itself in serving the community with respect and excellence. We ask that all vendors adhere to this protocol. We reserve the right to terminate participation for bad conduct or distasteful behavior before or during the event.
  • Safety for staff, vendors, and the community is a priority.
  • At tear down, please leave exhibit area clear and free of debris. Scattered disposable trash containers will be provided on site.

Vendors can choose from one of the following spaces: 1. Booth - a roughly 10' X 10' space in which a vendor can set up a 6' table. PLEASE NOTE: Vendors are to provide their own table, tablecloth and chairs. Optionally, they can set up a pop-up tent in the space. Or, 2. Mobile Unit space - a parking space large enough to park a vendor's Mobile Unit.
If using a booth, will you be bringing a pop-up tent?
Pop-up tents must be able to fit in a 10 X 10 space and will need weights as you will not be able to put stakes in the ground.
Would you like a representative from your organization to share a 60 second overview during the event, or would you prefer our DJ to share it?(Required)
